1) Mechanical breakdown occurs while chewing, but the stomach also churns the contents. Presumably it is mechanically broken down by the time it exits the stomach, but peristalsis in the intestines also performs a mixing action.
Chemical... depends on what is being broken down. Saliva contains amylase which breaks down sugars partially, the stomach contains pepsin which breaks proteins, and the intestine contains lipases, amylases, and peptidases (for fats, sugars and proteins).
2) Low stomach acidity... stomach enzymes do not work well at higher pH, so the food doesn't get broken down as completely in the stomach.
early postprandial dumping - happens with gastric bypass; food gets to the duodenum too fast and the contents are very acidic... therefore, the pancreastic enzymes that work here will not function as well because of the lower pH.
Gallstone - can block the release of bile into the duodenum.
3) Bile serves to emulsify fats (not break them down) which means that it takes makes the fats dissolve better in water... like dish detergent.
